There have been a number of questions recently regarding documentation to use to upload to FitLyfe or to prove you/your spouse have had a physical at ReforMedicine. Currently ReforMedicine and UMR are working to correct the EOB situation. We are also working with ReforMedicine to get a list of all employees/spouses that will qualify for the 2025 discount.
I have your information if you have done the following:
- You/your spouse completed your wellness appointments at Prevea prior to them closing
- You have uploaded and received points on the FitLyfe program
A few of you have provided me with some information or reached out to me for verification, I have started a list to verify your information once I receive the qualifying people from ReforMedicine.
